When the fundraiser is complete and all monies collected, the organizations check or money order will be made payable to Patricia Johnson.

Organization/Group will be responsible for the accuracy of the total amount of items sold and ordered on the Master Order Form.

Please allow 5-7 days for US Postal delivery time of Master Order Form and Payment. The candle delivery time is approximately 2-3 weeks from date both Master Order Form and payments are received.

Terms & Conditions:
This confirms that it is our intention to conduct this Candle Fundraiser during the dates specified. We understand that all personal checks will be made to our organization and in turn we will make one payment to Patricia Johnson, the Candle Fundraising Coordinator.

We enter into this agreement in good faith and should our organization be unable to conduct the proposed fundraiser, we are responsible for returning all sales material in the condition in which the materials were received by our group.  I understand and agree to pay for any unreturned sales material at .15 cents per quantity ordered.  If I do not notify The First Lady Boutique of our cancellation of the fundraiser and return the sales material within 30 days of said cancellation, I will be invoiced for the cost of the sales material.
